Phoenix VA Review Finds Problems

Department of Veterans Affairs inspectors find multiple issues at the Phoenix VA Health Care System. The agency uncovered deficiencies in safety processes, inspections, mental health diagnostics and geriatric medical evaluations. The Office of Inspector General issued 13 recommendations for the Phoenix VA to act on. The report was released just as President Trump is set to sign reform legislation Wednesday with the Director of the Phoenix VA in attendance.  Four years ago the agency was ground zero for the national crisis in health care for veterans. The Phoenix VA serves 80,000 vets.
